Athletic Pre-Participation Examination (PPE)
- A Colby-Sawyer Certified Athletic Trainer will review your medical history with you, conduct a concussion baseline exam (SAC), and then complete an orthopedic screening prior to participation in your sport. This appointment will be scheduled once all the paperwork is completed.Â
- All student-athletes will complete a computerized baseline Concussion Vital Signs (CVS) test for concussion their first year.Â
Pre-Participation Paperwork
Colby-Sawyer College uses an electronic medical records system (Athletic Trainer Software or ATS). There are a number of documents that will need to be completed in order to participate in intercollegiate athletics. These documents can only be found within the ATS system. All athletes (new and returning) must login to ATS to enter their demographic information and complete the required forms, including a health history and uploading your insurance card. Returning athletes will be required to login to ATS annually to fill out any necessary paperwork, update their health history and upload their insurance card.Â
